New Client forms will be sent to you when you schedule your appointment via email link. Additional forms must be downloaded and completed here.
New Client Form
Clients will be emailed a link to the New Client Intake form when scheduling an appointment, which covers your basic medical information and bodywork needs. This form can be completed on a computer or phone.

Physician Approval Form
If you are currently in treatment, between treatments, or if your last treatment for cancer was within one year of the date of the massage session, please download this form, have your physician fill it out, and return it at least 12 hours prior to your appointment.

Minor Consent Form
If the client receiving the massage is under the age of 18, this consent form must be downloaded, filled out from a parent/legal guardian, and returned before the appointment begins.
